It's unfortunately extremely dead. Over the last couple weeks I purchased the game, learned it, and pushed my character to lv96. In that time I've seen maybe 4 or 5 other people and that was only in the daily quest.
Subsequently, question is, do I need FFXV to play comrades? You need to have the main game in order to access the Comrades DLC. You don't need to finish the main game though. So long as it's there, you can play the DLC.
